Astellas Pharma Australia Pty Ltd is responsible for the Astellas Care Connect Program and for determining how personal information is handled for the Program. The Program is delivered on Astellas’s behalf by authorised service providers, including DKSH Patient Solutions. These service providers may collect, use, store and disclose personal information as necessary to administer the Program and meet applicable legal and regulatory requirements. Further information about Astellas’s privacy practices is available in the Astellas Privacy Notice.
The Program may collect personal information and sensitive information, including your name, contact details, date of birth, nominated Carer details, relevant health and treatment information, information provided by your healthcare professional, communications with the Program team, and information needed to manage safety reports. This information may be collected directly from you or, with appropriate authority or where legally permitted, from your nominated Carer, healthcare professional or another authorised person. If you do not provide information that is reasonably required, the Program may be unable to enrol you or provide some or all Program services.
The Program may collect, use and disclose your personal information to assess eligibility, completion of enrolment, provide Program services, communicate with you and/or with your nominated Carer or healthcare professional, respond to enquiries, improve Program delivery, manage safety information, comply with legal or regulatory obligations, and investigate or respond to privacy or security concerns.
The Program may disclose personal information to Astellas and its related entities, authorised Program service providers (including DKSH Patient Solutions), information technology and communications providers, healthcare professionals, professional advisers, regulators, government authorities and pharmacovigilance or safety-reporting recipients where reasonably necessary for the purposes described in these Terms or where authorised or required by law. Disclosures will be limited to information reasonably necessary for the relevant purpose. Where a disclosure is optional or outside the purposes explained when the information was collected, the Program will obtain further consent unless the disclosure is otherwise permitted or required by law.
Some Astellas entities, service providers, technology providers and regulatory or safety-reporting recipients may be located outside Australia, including in the USA. Personal information may therefore be accessed, stored, processed or disclosed in those countries. Before disclosing personal information to an overseas recipient, Astellas will take reasonable steps as required by applicable Australian privacy law to ensure that the information is handled consistently with applicable privacy requirements. Further information about likely overseas locations and applicable safeguards may be obtained by contacting Astellas using the privacy contact details below.
Personal information will be retained only for as long as reasonably necessary to administer the Program and meet applicable legal, pharmacovigilance, regulatory, dispute-management and records-retention requirements. When personal information is no longer required and no legal or regulatory obligation requires its retention, the Program will take reasonable steps to securely delete or de-identify it.
We may also deidentify or aggregate your information where necessary to comply with regulatory obligations.
Further details on how your information is handled, including access and correction rights, or how to make a complaint regarding a breach of privacy law, are set out in the Astellas Privacy Notice (available at astellas.com/privacy-center).
You may withdraw from the Program at any time by contacting the Program team through an approved communication channel. The Program team will process your request and provide written confirmation. From the time your withdrawal request is received, the Program will take reasonable steps to stop non-essential Program communications and service activities. Personal information may continue to be used, retained or disclosed where reasonably necessary to complete the withdrawal, maintain an appropriate record of your participation, address safety matters, comply with legal or regulatory obligations, or manage a complaint or dispute. Withdrawal will not affect your continuing medical care.
